Frye Regional’s Health Information Department helps you to obtain your medical record. Those requesting their own medical record must present a photo I.D. and be at least 18 years of age. In special circumstances, with the patient’s permission, records may be obtained by someone other than the patient. Surviving relatives or executors of the estate may also obtain the record if needed. The patient’s primary care physician or specialist(s) also have access to the patient’s record for continuing care.
